Support Coordination
The Arc Davidson County & Greater Nashville’s Independent Support Coordination (ISC) program employs twelve independent support coordinators who support three hundred sixty adults and children with intellectual disabilities in twenty counties in Middle Tennessee. ISC management and staff at The Arc of Davidson County & Greater Nashville have a combined total of over 100 years experience working with people with disabilities.
The coordinators help people to plan for and access medical, dental and therapeutic care and support services which contribute to their self-sufficiency and participation in home, work and community life. Unfortunately, our agency is prevented by Department of Intellectual and Developmental Disabilities rules from providing other waiver funded direct services such as: day programs, supported living homes or transportation while contracted to provide Support Coordination. However, this allows us to preserve the independent aspect of the ISC's case management role and allows us to advocate freely for the best possible options for the person. Adults and children with intellectual disabilities who qualify for the Medicaid Waiver are eligible.
The program is funded through a provider agreement with the Tennessee Department of Intellectual and Developmental Disabilities.
Of the program’s many accomplishments, the one of which ISC program staff are most proud is our continued collaboration and participation with the Tennessee Department of Intellectual and Developmental Disabilities in the development and promotion of person centered practices and policies throughout the state.
Independent Support Coordination Update
The State of Tennessee Department of Intellectual and Developmental Disabilities (DIDD) has been working for several years to establish a comprehensive quality management system to measure the quality of services and supports provided by community-based providers.
We are more than happy to report that The Arc Davidson County & Greater Nashville’s Support Coordination program has received “Substantial Compliance” scores, an “Exceptional Performance” quality assurance performance rating and has been awarded a “Four-Star Ranking” (the highest level awarded based on all criteria in the State review process) for the third consecutive year!
As a result of this ranking, the State of Tennessee has determined that The Arc is exempt from program monitoring for the remainder of 2016.
